Job description

Position Summary:

The Community Health Manager leads the Alzheimer's Association's implementation of the Idaho Cognitive Health Workforce Network, funded through Idaho's Rural Health Transformation Program. As lead entity in a hub-and-spoke model, the Association chairs the Network's steering committee, oversees sub-awards to partners across the state, and works with academic, clinical, and community partners to build a dementia-capable workforce in rural and frontier Idaho.

This role strengthens the ability of primary care providers, nurses, care coordinators, community health workers, social service professionals, direct-care aides, and health professions students to recognize early cognitive decline, connect patients and family caregivers to dementia care and community resources, and engage caregivers in care through organizations and networks outside of the Association. The Manager builds and sustains relationships with state agencies, health systems, FQHCs, universities, and community organizations, and ensures the Network meets its goals, deliverables, and reporting requirements. The components of this grant include:

  • Network Infrastructure and Grant Administration (steering and advisory committees, sub-awards, technical assistance, and evaluation and reporting)
  • A Dementia-Capable Workforce Development (rural workforce needs assessment, Idaho Dementia Workforce Hub, clinical training pipeline, and Project ECHO telementoring)
  • Statewide Referral System Exploration (connecting health systems and community organizations to dementia care, education, tools, and caregiver resources)

Note: Specific priorities and activities are defined by the Idaho award and may be adjusted in coordination with the Idaho Department of Health and Welfare. This is also a grant-funded position.

Essential Duties and Responsibilities:
  • Lead day-to-day implementation of the Idaho Cognitive Health Workforce Network across its three components (Network infrastructure and grant administration, dementia-capable workforce development, and statewide referral system exploration), ensuring grant requirements, deliverables, timelines, and key performance objectives are met.
  • Establish, convene, and staff the Network’s steering committee (chaired by the Association) and advisory committee, including adoption of a Network charter, ensuring non-duplication across activities, and connecting the Network to the Department and existing statewide ADRD workgroups and initiatives.
  • Oversight of technical assistance and coordination to sub-awardees and partners through supervision of a state technical assistant staff, including monthly technical assistance calls, and manage post-award monitoring in partnership with the Network’s Technical Advisor.
  • Oversee Network workforce activities, including the rural workforce needs assessment with Upstream Thinking, development and promotion of the Idaho Dementia Workforce Hub, expansion of rural clinical training and pipeline opportunities through AHEC, and the Project ECHO Idaho ADRD series.
  • Build and sustain partnerships with FQHCs, health systems, clinics, rural health providers, universities, Area Agencies on Aging, Tribal and community organizations, professional associations, and state workforce and aging agencies to drive outcomes related to grant requirements.
  • Plan and host the annual in-person convening of Network partners and advance sustainability of the Network beyond the grant period.
  • Lead and coordinate the Association staff supporting the Network, including the Technical Advisor and the Associate Director of Applied Research & Program Evaluation, in collaboration with the Senior Director of Professional Education & Workforce Development and the Director of Psychosocial Research and Program Evaluation.
Knowledge, Skills and Abilities
  • Dementia Care, Rural Health, Health Systems & Public Health Expertise
  • Familiarity with interdisciplinary professional roles in rural and frontier populations, such as community health workers (CHW), social workers, nurses, physicians, care coordinators, and direct-care aides
  • Proven ability to lead large-scale state program implementation within health, academic, or community-based systems
  • Demonstrated ability to build and sustain strategic partnerships with state agencies, universities, health systems, FQHCs, rural health organizations, public health agencies, Tribal communities, and other community partners
  • Experience developing and delivering workforce education, training, and capacity-building initiatives, including telementoring, continuing education, and clinical training pipelines
  • Ability to manage budgets, timelines, sub-awards, contracts, and federally funded grant requirements
  • Experience facilitating committees and coalitions, and communicating effectively with leaders across clinical, care, public health, academic, and community sectors
  • Experience supervising, coaching, and developing a high-performing team
  • Strong project management skills with the ability to prioritize, delegate, and coordinate multiple workstreams and partners
  • Proficiency in using data systems, dashboards, and reporting tools to track program performance, outcomes, and key performance objectives
  • Strong written and verbal communication skills, including the ability to translate complex concepts for diverse audiences
  • Knowledge of Idaho’s rural and frontier health and aging landscape preferred
REQUIRED EDUCATION AND EXPERIENCE

Bachelor’s degree required in public health, social work, health administration, nursing, gerontology, or related field.

Master’s degree preferred in Public Health (MPH), Social Work (MSW), Healthcare Administration (MHA), Gerontology, or related discipline.

Experience:
