Job summary

Sorogi Health in Washington, DC seeks a compassionate Community Health Worker to support families across SorogiEAT, programs, and the Foundation. You will walk with participants from enrollment through success, providing health education, coaching, and advocacy.

You will bridge communication between participants and Sorogi staff, monitor engagement, coordinate visits, and document progress while upholding Sorogi’s dignity standard.

Qualifications

• High school diploma or equivalent required; CHW training or certification is a plus.
• Experience in health education or coaching for chronic conditions is preferred.

Responsibilities

• Advocate for participants to ensure they receive the best possible care and program satisfaction.
• Bridge communication between participants and Sorogi staff by monitoring participation and consistently improving engagement.
• Present Sorogi’s programs and services to participants and confirm they know whom to contact with questions or concerns.
• Prevent and resolve complaints: flag emerging problems early, suggest fixes, and help modify practices that lead to recurring issues.
• Perform administrative tasks, including calls and text messages via Sideline, scheduling participant visits, and overseeing the daily program schedule to ensure efficiency.
• Conduct and document monthly participant questionnaires and feedback.
• Participate in recruitment and community outreach sessions and follow up with potential participants.
• Coordinate SorogiEAT produce deliveries and engage families without judgment.
• Onboard participants onsite and virtually and train them on portal usage.
• Collect and monitor health data from participants with diabetes and hypertension and securely transmit to providers.
