Job summary

The Community Health Worker (CHW) at Sanford Health will serve as a bridge between under-served communities and the health and social service systems, operating in Marshfield, WI, within a patient-centered medical home model to support chronic disease self-management.

You will provide information and navigation assistance, advocate for patients and community groups, and collaborate with an inter-professional team to plan care and coordinate appointments.

Qualifications

• Motivational Interviewing skills required with training, to be completed within six months of hire.
• High school diploma or equivalent required; college courses strongly preferred but not required.
• Proof of certificate from a Community Health Worker program is required upon hire or within six months.
• Must have a valid driver’s license.

Responsibilities

• Act as a bridge between underserved communities and health and social service systems.
• Work within the patient-centered medical home model with a team approach to chronic disease self-management.
• Provide information and assistance to help patients navigate a complex health and human services system.
• Give presentations to community groups and engage in public speaking with diplomacy and tact.
• Coordinate appointments and individual plans of care within an inter-professional team.
• Maintain privacy and boundaries and advocate for patients and community groups.
