Job summary

Squirrel Hill Health Center is seeking a Family Nurse Practitioner (FNP-C or FNP-BC) to provide independent primary care across all ages, including prenatal support with OB/GYN collaboration. The provider will visit SHHC locations, mobile units, and homes as needed, delivering trauma-informed, culturally competent care in a community-focused setting.

Qualifications

Advanced degree and certification from an accredited Nurse Practitioner program.
PA license to practice as an APRN without restrictions.
Active DEA registration or eligibility to obtain.
Participation in Medicare and Medicaid programs.
Experience with EHR systems and care coordination.
Ability to work evenings and across SHHC locations.

Responsibilities

Conduct patient assessments, physical exams, diagnose acute and chronic conditions, and develop individualized treatment plans.
Orders or performs tests and imaging to inform patient care.
Analyzes test results and examinations to diagnose conditions.
Manage chronic diseases in geriatric populations; vaccines and preventive care.
Coordinate care with specialists, behavioral health, social workers, and community resources.
Document care and billing in electronic health records within three working days.
Take rotating after-hours call with other clinicians as directed.
Provide culturally competent, trauma-informed care to diverse populations.
