Job summary

Disability Supports Of The Great Pl in McPherson, KS is hiring a full-time caregiver to provide quality living and community support. Duties include implementing person-centered support plans, assisting with daily living skills, administering medications, and completing required documentation.

You will collaborate with program managers and wellness liaisons, attend meetings, and may drive agency vehicles. Normal hours are 7:00 am to 4:00 pm, Monday through Friday, with occasional duties as

Qualifications

• Experience in direct care is preferred but not required.
• High school diploma or equivalent preferred.
• Strong communication and compassionate approach required.
• Must be able to lift up to 50 pounds and assist with daily living activities.
• Valid driver's license preferred.

Responsibilities

• Implement Person Centered Support Plans for assigned individuals.
• Assist with daily living skills including meal prep, hygiene, toileting, housekeeping, activities.
• Administer medication and treatments.
• Complete documentation as required.
• Report incidents to Program/Wellness Manager.
• Attend required meetings.
• If driving, may transport individuals.
• Hours are 7a - 4p Monday - Friday.

Role: To provide quality living and community support and be responsive to individuals needs in residential, work or community settings. Teach, support and facilitate choices for individuals in their homes, work or community settings.

Major Duties and Responsibilities:

• Implement Person Centered Support Plans for assigned individuals.
• Assist with daily living skills including, but not limited to, meal preparation, general hygiene, toileting, general housekeeping, work and leisure activities.
• Administer medication and treatment(s).
• Complete documentation as required through the general course of duties.
• Report to the appropriate Level 2 or Level 3, Program Manager, Case Manager or Wellness Liaison any occurrences that affect the immediate well-being of the individual.
• Attend all meetings as assigned and required.
• If approved as a driver of agency vehicles, employee may transport individuals.
• Hours for this position are 7a - 4p Monday - Friday.
• Other duties as assigned.

QualificationsKnowledge and Skills:

• Experience: Prior direct care experience preferred, but not required.
• Education: High School diploma/equivalent preferred.
• Interpersonal Skills: Must be able to communicate effectively with all levels of personnel and individuals served. Must demonstrate compassion for individuals with disabilities.
• Other Skills: Basic mathematical, grammar and computer skills preferred. Valid drivers license preferred.
• Physical/Environmental: Must be able to lift, push and/or pull a minimum of 50 pounds, stand for periods of time, bend, push wheelchairs, and provide physical intervention on a regular basis. Noise level may vary, depending upon duty assigned.

Due to the potential intimate nature of care and physical proximity involved with some of those receiving HCBS funded I/DD services through Disability Supports, and in order to promote a safe work environment for both the caregiver and those for whom we care, you may, from time-to-time, be required to wear PPE, (Personal Protective Equipment). If PPE is required, Disability Supports will provide it.
