Job summary

Kaleidoscope Inc. is hiring a Community Clinician to support youth and families in the ECCS program across Chicago in a hybrid role. The position involves direct clinical work, caregiver coaching, consultation, and advocacy with a caseload of 7-10 youth.

Requires a Master's in Social Work or related field; licensure (LCSW/LCPC) preferred. Travel across Cook County for in-person sessions is required. A $3,000 signing bonus and strong supervision are included.

Qualifications

Responsibilities

  • Provide direct clinical support and consultation to caregivers, caseworkers, and teams for a caseload of 7-10 youth.
  • Conduct weekly in-person caregiver support sessions focusing on trauma-informed psychoeducation and parenting strategies.
  • Organize and facilitate monthly clinical team meetings with youth, caregivers, caseworkers, and providers.
  • Maintain communication with agencies and partners to identify needs and barriers.
  • Collaborate to develop, review, and update individualized service plans.
  • Utilize trauma-informed clinical models (e.g., ARC, SPACE, SMART, Theraplay).
  • Provide ongoing clinical consultation to caregivers and professionals.
  • Support families with clinical case management and referrals.
  • Advocate for youth and family needs in staffings, meetings, and systems spaces.
  • Ensure accurate documentation: progress notes, plans, CANs 2.0, discharge summaries.
  • Consult with supervisors to ensure safety and well-being.
  • Travel throughout Cook County for in-person sessions.

This is a field-based, hybrid position. You must live in Illinois and be able to travel throughout Cook County to provide in-person services to youth and families in homes and community settings.

We are expanding our Enhanced Clinical Community Support (ECCS) Program and are hiring Community Clinicians to join this highly collaborative, mission-driven team.

About The ECCS Program

The Enhanced Clinical Community Support (ECCS) Program provides immediate, short-term, intensive clinical services to stabilize youth in DCFS care following inpatient psychiatric hospitalization. Services are delivered in the home and community and focus on strengthening caregiver capacity, improving system coordination, and reducing the risk of placement disruption.

About The Role

The Community Clinician is a unique, advanced clinical role that blends direct clinical intervention, caregiver coaching, consultation, and advocacy. Clinicians partner closely with caregivers, caseworkers, and multidisciplinary teams to ensure continuity of care and developmentally appropriate, trauma-informed support for youth.

What You'll Do
  • Provide direct clinical support and consultation to caregivers, caseworkers, and multidisciplinary teams for a caseload of 7-10 youth
  • Conduct weekly in-person caregiver support sessions focused on trauma-informed psychoeducation and therapeutic parenting strategies
  • Organize and facilitate monthly clinical team meetings with youth, caregivers, caseworkers, and other involved providers
  • Maintain consistent communication with assigned casework agencies and collateral partners to identify youth needs, family needs, and system barriers
  • Collaborate with child and family teams to develop, review, and update individualized service plans
  • Utilize trauma-informed and evidence-based clinical models, including ARC, SPACE, SMART, Theraplay, or other appropriate interventions
  • Provide ongoing clinical consultation to caregivers and professionals supporting the youth
  • Support families with clinical case management, including referrals for therapy, psychiatry, and other services
  • Advocate for youth and family needs in DCFS clinical staffings, CFTMs, school meetings, and other systems spaces
  • Ensure timely and accurate documentation, including progress notes, treatment plans, CANS 2.0, and discharge summaries
  • Consult regularly with supervisors and team members to ensure youth safety, permanency, and well-being
  • Travel throughout Cook County for in-person sessions with families
Schedule
  • Flexible full-time schedule
  • Evening availability required (e.g., 10:00am-6:00pm or 11:00am-7:00pm)
  • Hybrid role with a combination of community-based and remote work
What You Need
  • Master's degree in Social Work or a related social service field
  • LCSW or LCPC preferred (not required)
  • Experience working with youth involved in child welfare and/or complex trauma
  • Strong clinical judgment, organization, and communication skills
  • Comfort using Microsoft Word, Outlook, and video conferencing platforms
  • Valid driver's license, insured vehicle, and ability to travel throughout Cook County
