Job summary

The role involves arranging in-home setup, addressing barriers to safe placement, and maintaining detailed documentation and reporting. Regular travel to community locations is expected.

Qualifications

• Requires a Bachelor's degree and 2 – 4 years of related experience.
• Or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position.

Responsibilities

• Supports transition planning by coordinating with care teams, providers, members, families and caregivers.
• Arranges essential household items, furnishings, groceries, and in-home setup for transition.
• Maintains accurate documentation, tracking and monthly reporting of transition activities.
• Assists with developing education and training programs for care coordination staff and providers to improve transition services.
