Job summary

Cap City Supported Living & Home Care Services provides care to young adults and adults with developmental disabilities, offering social, community-based programs and home personal care in residential settings.

The role involves direct client care, assistance with daily tasks, safety, transportation, medical coordination, and precise documentation as required by the agency and regulations.

Qualifications

• High School Diploma or equivalent is required.
• Valid Driver's License with no more than 6 points.
• Current Proof of auto insurance (state minimum).
• 18 years of age or older.
• FBI and/or BCI Background Check required.
• CPR and First Aid Training required.
• 8hr DODD Training completed or to be completed.
• Medication Administration Training completed or to be completed.

Responsibilities

• Provide direct care to clients per their ISP and needs.
• Coach/assist with routine tasks, housekeeping, meal prep, and personal care.
• Coach/assist with safety checks and emergency response.
• Assist with day-to-day finances and access to community activities.
• Schedule, attend medical appointments, fill prescriptions, and assist with medications.
• Provide safe transportation for social, medical, and general needs.
• Attend meetings and complete documentation accurately per requirements.
