Job summary

The Diocese of Camden seeks a Care Coordinator for full-time work in Camden County and surrounding areas. This exempt role is 35 hours per week and reports to the Director, Care Coordination and Consultation.

You will perform in-home and community assessments, develop individualized care plans, coordinate services, and advocate for patients while ensuring HIPAA confidentiality. Bilingual English/Spanish preferred.

Qualifications

• RN or MSW with current New Jersey license and CPR certification.
• Minimum of 3 years health care experience, preferably in community/home care, care coordination, discharge planning or case management.
• Highly organized and ability to work autonomously.
• Bilingual preferred (English/Spanish).
• Excellent communication skills required.

Responsibilities

• Provide care coordination for individuals in the community.
• Establish effective relationships with patients, families, professionals, payers and others.
• Conduct face-to-face assessments in home, community or medical settings.
• Educate patients and families to understand services and self-management.
• Develop individualized care/service plans and monitor progress.
