Job summary

Integrated Services for Behavioral Health (ISBH) seeks a Housing Community Behavioral Health Worker to provide in-home and community-based support in Athens County, OH. You will empower individuals and families to build routines and coping strategies while connecting them to local resources and partners.

The role emphasizes trauma-informed, strength-based care and engagement with clients and caregivers. The position offers hourly pay ranging from $19.00 to $22.26, with benefits including

Qualifications

  • High School diploma or GED required.
  • Bachelor’s degree in a related field or minimum 3 years of direct relevant experience.
  • Experience with multi-need individuals and families preferred.
  • Experience partnering with local behavioral health systems, housing, legal systems, and social services preferred.
  • 3+ years of experience in related field preferred.

Responsibilities

  • Engage individuals and families in home and community settings to identify strengths, barriers, and goals.
  • Provide skill building, coaching, and practical supports in home, school, and community settings.
  • Help clients develop routines, coping skills, emotional regulation, and healthy communication.
  • Assist clients in navigating community resources through modeling and coaching.
  • Deliver care in home and community-based settings.
  • Collaborate with internal programs and external partners to ensure quality of care.
  • Maintain ongoing communication with clients, caregivers, and team members.
  • Document services in EHR and adhere to policy.
  • Meet productivity and quality standards and HIPAA requirements.
  • Participate in team meetings and required trainings.
  • Travel within designated service areas.

We are seeking a Housing Community Behavioral Health Worker!

Athens County, OH

Join our Team! Integrated Services for Behavioral Health (ISBH) is a community-minded, forward-thinking behavioral health organization helping people along the road to health and well-being. We meet people in their homes and communities and help connect them to the resources they need. We serve southeastern and Central Ohio with a comprehensive array of behavioral health and other services – working with local partners to promote healthy people and strong communities. All of our services are intended to be collaborative and personalized for the individual.

The Community Behavioral Health Worker provides direct services that empower individuals and families to build skills and overcome barriers to behavioral health and daily functioning. The CBHW provides coaching and practical support in home, school, and community settings, helping clients develop routines, coping strategies, and healthy communication. This role guides access to community resources, collaborates with internal teams and external partners, and maintains strong engagement with clients and caregivers. All services are delivered using trauma-informed, and strength-based approaches to promote recovery and resilience.

The pay range for this position is $19.00 to $22.26 an hour based on experience, education, and/or licensure.

Essential Functions:
  • Engage individuals and families in the home and community to identify strengths, barriers, and goals related to behavioral health and daily functioning
  • Provide skill building, coaching, and practical supports in home, school, and community settings
  • Support clients in developing routines, coping skills, emotional regulation, executive functioning skills, and healthy communication patterns
  • Assist clients in navigating community resources through modeling, coaching and teaching
  • Deliver care in home and community-based settings
  • Collaborate with internal programs and external partners to ensure quality of care
  • Maintain consistent communication with clients, caregivers, and team members to support engagement and follow through
  • Utilizes databases and Electronic Health Record systems for accurate client documentation
  • Adhere to ISBH EHR documentation policy
  • Meet ISBH standards for productivity and quality of service delivery
  • Maintain compliance with HIPAA, mandated reporting requirements, and professional ethical guidelines
  • Participate in team meetings, supervision, and required training to support ongoing development
  • Use trauma-informed, strengths-based approaches in all client and team interactions
  • Travel within the designated service areas
  • Perform other duties as assigned
Minimum Requirements:
Education:
  • A high School diploma or GED equivalent is required
  • A bachelor’s degree in related field or minimum 3 years of direct relevant experience is required
Experience:
  • Experience with multi-need individuals and families is required
  • Experience partnering with local behavioral health systems, housing, legal systems, and social services is preferred
  • 3+ years of experience in related field is preferred
Knowledge, Skills and Abilities:
  • Broad knowledge of community service systems is required
  • Advanced communication skills, both oral and written is required
  • Advanced organizational skills are required
  • Ability to bill Medicaid without restrictions is required
  • A valid driver's license and the ability to operate a motor vehicle is required
  • An appropriate level of auto insurance coverage is required
  • Ability to use a personal vehicle or company vehicle to provide services in homes, schools, and community settings is required
  • Ability to maintain confidential information is required
  • Ability to manage deadlines within a fast-paced, high-volume environment is required
  • Ability to operate in an Internet-based, automated office environment is required
  • Ability to maintain high-speed internet connection is required
Physical Requirements:
  • Prolonged periods of sitting at a desk and working on a computer is required
  • Must be able to lift up to 15 pounds at times is required
