Job summary

Hoyleton Youth & Family Services is seeking a full-time Behavioral Health Specialist based in Fairview Heights, IL. The role provides community-based mental health rehabilitation and outreach to children and adults with complex needs, following Medicaid rules and supporting recovery goals.

Responsibilities include outreach, assessments, care planning, and collaboration with the clinical team, with a focus on trauma-informed approaches and crisis de-escalation principles under TCI and CARE

Qualifications

• Bachelor’s Degree required; 2+ years in the human services field preferred.
• Must possess and maintain a valid driver’s license and auto insurance.
• Ability to receive a DCFS/CANTS Clearance.

Responsibilities

• Outreach to at-risk individuals and link to resources.
• Conduct trauma-informed assessments to inform treatment planning.
• Help clients develop adaptive functioning to stay in the community.
• Promote client participation and engage family and support networks.
• Maintain case documentation and billing compliant with Medicaid rules.
• Collaborate with Clinical Supervisor and participate in case staffing.
• Attend trainings and participate in community events as needed.
• Support CARE and TCI program requirements.
