We are seeking a compassionate and skilled Community-Based Therapist to join our team in Port Saint Lucie, FL. The ideal candidate will provide therapeutic services to individuals and families within the community and homes, focusing on improving mental health and overall well-being through evidence-based practices.

Key Responsibilities

• Provide individual, family, and group therapy sessions in community settings.
• Conduct comprehensive assessments to identify client needs and develop personalized treatment plans.
• Collaborate with community organizations, schools, and healthcare providers to coordinate care and support services.
• Maintain accurate and timely documentation of client progress and treatment outcomes.
• Utilize culturally sensitive approaches to engage diverse populations.
• Participate in case conferences and multidisciplinary team meetings.
• Promote mental health awareness and education within the community.

Qualifications

• Master’s degree in Social Work, Psychology, Counseling, or related field.
• Valid licensure as a therapist or Registered Intern (LCSW, LMHC, LPC, or equivalent) in the state of Florida.
• Experience providing community-based therapeutic services preferred.
• Strong knowledge of evidence-based therapeutic techniques and interventions.
• Excellent communication and interpersonal skills.Ability to work independently and as part of a multidisciplinary team.
• Reliable transportation to travel within the community.

Essential Functions

• Conduct initial assessments of client needs and eligibility for services.
• Develop individualized treatment plans which fully address the client's needs.
• Provide ongoing evaluation, assessment, and direct clinical services to assigned clients, using appropriate therapeutic techniques.
• Coordinate with schools, community agencies, and other service providers to ensure provision of effective and appropriate services.
• Participate in court proceedings, clinical staffings, and education meetings as needed.
• Coordinate and/or provide crisis intervention as needed.
• Complete all required clinical documentation regarding course of treatment within designated time frame, ensuring documentation meets quality and compliance requirements.
• Consistently update client files to ensure compliance with Medicaid, payer, and company guidelines.
• Consistently conduct a minimum of billable hours per week as established for the position (30 billable hours for full-time status and 15 hours for part-time status).

WORK ENVIRONMENT

This job operates in a professional office environment in a controlled atmosphere building.

This job also works clients homes and within the community, where the atmosphere is not controlled by the employer. This position will also spend time traveling in their personal vehicle.
