Qualifications

• Active Minnesota PA-C license in good standing.
• Experience in geriatrics, family medicine, internal medicine, home health, hospital medicine, post-acute care, or related settings preferred.
• Strong communication skills and confidence working collaboratively with interdisciplinary teams.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide on-site primary care and rounding services across assisted living, memory care, and long-term care communities.
• Perform assessments, chronic disease management, preventative care, and follow-up visits.
• Develop and adjust individualized care plans.
• Coordinate care with physicians, nurses, therapists, caregivers, and family members.
• Support transitions of care and help reduce avoidable ER visits and hospitalizations.
• Ensure continuity of care across a diverse patient population.
