The role supports participants in navigating employment pathways with personalized guidance in a community-based setting.

The position involves managing a caseload, coordinating with worksites and residential supports, and providing job coaching and support while documenting progress

Responsibilities

• Provide case management services to assigned participants.
• Assist participants with career exploration, job seeking, interviewing, and coaching.
• Coordinate with worksite supervisors and residential supports to ensure successful employment.
• Maintain accurate progress records in compliance with organizational policies.
• Develop and implement support strategies to accomplish ISP and MabVoc goals.
• Prepare participants for work experiences and follow-up meetings.
