Job summary

Integrated Services for Behavioral Health (ISBH) seeks a Care Coordinator to link families with services and support children in home, school, and community settings in Ohio. You will collaborate with Child and Family Teams to coordinate services, identify community supports, and ensure plans meet safety and treatment needs while promoting family voice and choice.

Strong knowledge of behavioral health, wraparound, and community resources is preferred, with willingness to learn and lead

Qualifications

  • Experience providing services to children and families in behavioral health or related public sector human services.
  • Knowledge of wraparound/crisis coordination and community resources preferred.
  • Ability to work with diverse populations and manage multiple tasks.

Responsibilities

  • Identify care coordination needs with families to support child outcomes.
  • Coordinate family-based services across home, school, and community.
  • Ensure services align with safety, treatment needs, and family voice.
  • Build partnerships with community resources for wraparound supports.
