Job summary

Abode Care Partners seeks a clinically oriented physician to join a clinical collaborative role supervised by the CMO. You will provide day-to-day geriatric and complex care across facilities and home-bound patients, while modeling ethical and evidence-based decision-making for staff.

The role involves leadership in administration, collaboration with nurse practitioners/physician assistants, and participation in growth initiatives within the senior living network.

Qualifications

• Graduate from an accredited medical school with Doctor of Medicine.
• Active medical license in the state of employment and DEA registration.
• Board certification in Family Medicine, Internal Medicine, or Geriatrics; geriatrics preferred.
• Two+ years of administrative leadership experience in healthcare; seven+ years clinical experience preferred.

Responsibilities

• Provides high-quality complex and geriatric care to residents and home-bound patients.
• Engages with facility leadership, nurses, patients and families.
• Mentors nurse practitioners and physician assistants; collaborates to support clinical roles.
• Audits practitioner orders and documentation monthly as per state/regulatory rules.
• May assume medical directorships for facilities as needed.
• Identifies growth opportunities in skilled and senior living facilities.
• Participates in frontline/backup call processes for timely acute changes to care.
• Focuses on quality outcomes, care path adherence, and cost reduction.

Overview

In this clinical collaborative physician role, under supervision of the CMO, you will have several clinical responsibilities, along with some specified administrative responsibilities. While providing day-to-day clinical care to residents residing in facilities as well as home bound patients, you will practice and role-model ethical and evidence-based decision-making to promote the vision and mission of the company to other practitioners and staff.

Responsibilities

• Provides high-quality complex and geriatric care to residents and patients across settings in partnership with non-physician practitioner and other interprofessional team members to an identified panel of patients across various assisted living and skilled nursing facilities
• Keeps high engagement with facility leadership teams, nurse practitioners/physician assistants, patients, and families
• Provides collaborating physician duties for on average of 5 but up to 15 nurse practitioners and physician assistants (based on state regulatory parameters) to support them in their clinical roles
• Based on state and regional regulatory frameworks, audits up to 10% of practitioner medical orders and/ or documentation for each provider monthly
• If needed, assumes medical directorships for skilled and/or other senior living facilities
• Helps identify new growth opportunities with skilled facilities and/or other senior living facilities in the market/region
• Participates in company frontline /backup call processes for timely response to acute changes of condition to prevent avoidable hospital transfers, or to meet other clinical needs
• Keeps a high focus on quality outcomes including but not limited to care path compliance and productivity, prevention of avoidable transfers and decrease in polypharmacy and costs of care
• Attends required trainings, and meetings as mandated by the CMO or other leaders to assure best care delivery by the company

Qualifications

Education/Experience:

• Graduate from an accredited school of medicine and is a Doctor of Medicine
• Two or more years of administrative leadership experience in healthcare
• Clinical medical experience of seven years or more preferred

Certifications/License/Other Special Requirements

• Board certification in Family Medicine, Internal Medicine or Geriatrics; qualification in Geriatrics preferred
• Practicing physician currently licensed under the provisions of the State Board of Medicine
• Active unrestricted licensure to practice in state of employment
• Current Drug Enforcement Administration registration
