Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Minimum 2+ years RN experience; outpatient setting preferred.
• NH RN License; BLS within 3 months; CDCES preferred but not required.
• CDCES within 1 year of hire.

Responsibilities

• Apply nursing process for assessment, planning, delivery and evaluation.
• Coordinate with Primary Care Providers and care team.
• Utilize health coaching for self-management of chronic illness.
• Assist with Advance Directives, Wellness Visits & chronic disease plans.
• Serve as a resource and support for team members.
