Position Summary

The Licensed Integrated Mental Health Therapist-BHN is responsible for independently providing assessments, treatment/care planning and psychosocial therapy within a primary care clinic setting for a diverse population of preventative, acute care and chronic disease management patients. The incumbent uses a variety of theoretical treatment models and interventions and excellent communication skills to work within a primary care/chronic disease management team setting with limited supervision; documents treatment/communication in keeping with department standards and keeps current on community resources to effectively provide linkage and advocacy to fulfill client's needs.

Responsibilities

• Assesses patient’s symptoms and creates a customized treatment plan based on individual patient needs.
• Screens patients for common behavioral health disorders using PHQ‑9, GAD‑7, Columbia Suicide Risk Assessment, or other tools as required.
• Monitors patients for changes in clinical symptoms and treatment side effects or complications, with a goal of reduction in symptoms.
• Supports psychotropic medication management prescribed by PCPs, focusing on treatment adherence, side‑effects, and treatment effectiveness.
• Tracks patient follow‑up and clinical outcomes using a registry, and uses this system to identify and re‑engage patients.
• Facilitates care plan changes for patients who are not improving, including changes in medications or psychosocial treatments or appropriate referrals for additional services.
• Manages behavioral health crisis as they arise in a primary care setting.
• Provides flexible behavioral health services to patients on and off current caseload, both through scheduled appointments and immediate interventions via warm handoffs during medical visits with primary care providers.

Requirements

• Master’s degree from an accredited institution in Social Work, Counseling, Marriage & Family Therapy, or a related field.
• Licensed Marriage & Family Therapist (LMFT), Licensed Mental Health Counselor (LMHC), or SW Independent Clinical Lic (LICSW).
• Minimum two (2) years of experience in a behavioral health setting.

Potentially up to $5,000 sign on bonus!

Potentially up to $10,000 relocation bonus!

Pay is influenced by factors specific to applicants, including but not limited to skill set, level of experience, and certifications or education. If this position is associated with a union contract, pay will reflect the appropriate step on the pay scale based on the applicant’s years of experience.
