Job summary

Trinity Health IHA Medical Group is seeking a compassionate Behavioral Health Specialist to join our Behavioral Health Collaborative Care team in Michigan. You will assess, educate, and monitor patients with behavioral health concerns, coordinating care with primary care providers and psychiatric consultants to improve outcomes.

The role emphasizes brief evidence-based interventions, telehealth and in-person visits, and ongoing symptom monitoring to enhance patient experience and treatment

Qualifications

• Master’s degree or higher in Social Work, Counseling, or related behavioral health field.
• Valid Michigan clinical licensure (LMSW, LPC, LMFT, or similar applicable licensure).
• Experience assessing/treating common mental health or substance use disorders; familiarity with CBT/PST/Behavioral Activation preferred.

Responsibilities

• Coordinate behavioral health care with primary care providers, psychiatric consultants, and other treating providers.
• Screen and assess patients for mental health and substance use disorders.
• Provide patient education regarding behavioral health conditions and treatment options.
• Monitor patients for changes in symptoms, treatment response, and medication side effects via in-person/telehealth.
• Support psychotropic medication management by promoting adherence and identifying barriers.
• Deliver brief evidence-based interventions such as Behavioral Activation and other structured approaches.
• Document and track patient outcomes and encounters.
