Job summary

Adventist Health Tillamook seeks a licensed physical therapist to join our team, working within a multidisciplinary approach to restore and improve physical, social, and mental functions. You will lead evaluations, design treatment plans, and coordinate care with other clinicians.

The role requires a DPT and state licensure, with CPR/BLS certification. Prior PT experience is preferred; duties include documentation, patient education, and supervising assistants in a community-based setting.

Qualifications

• DPT degree required; license after Jan 2016 considered; foreign equivalent.
• Two years' Physical Therapy experience preferred.
• Physical Therapy licensure in state of practice required.
• CPR/BLS certification required.

Responsibilities

• Completes evaluations to ensure quality standards and addresses assessment areas.
• Provides therapy interventions using appropriate treatment protocols and techniques.
• Establishes individualized care and discharge plans and communicates them to relevant parties.
• Completes comprehensive and accurate clinical documentation within established deadlines.
• Supervises assigned assistants.
• Performs other job-related duties as assigned.
