Responsibilities

Position Description:

  • Conduct assessment including review of referral information, identifying and engaging key participants, identifying systemic risks and protective factors, and developing an analysis of the problem behaviors within the ecological context and model process.

  • Engage primary caregiver and other key participants in active change-oriented treatment by identifying and overcoming barriers to engagement and generalization of treatment.

  • Implement a problem conceptualization, identify community based resources to provide family specific intervention, monitor community based intervention an d outcome review and strategy revision procedure using the model's process.

  • Maintain clear, concise and timely documentation of intervention efforts that promote peer and supervisory review and feedback, and that demonstrate compliance with model requirements.

  • Maintain clear, concise and timely documentation of intervention efforts and Family Assessment and Service Planning according to the State Connection System requirements.

  • Collaborate with all relevant systems and key participants within each system to ensure their buy-in and cooperation throughout treatment.

  • Participate in all model program training, supervision and consultation activities.

  • Provide home-based interventions.

  • Report to assigned team supervisor

  • Maintain a maximum caseload, consistent with model expectations.

  • Adhere to model expectations and implement with fidelity.

  • Follow the Key Performance Indicators set by program and follow through with clear next steps

  • Complete Family Treatment Plans, as needed and per model expectations.

  • Weekend availability required, based on client needs.

  • Ability to travel outside of assigned borough/region based off of special language skill/s and client needs.

  • Ability to travel outside of assigned borough/region if additional support is needed.

  • Conduct intakes as needed.

  • Assist with on-c all rotation.

  • Regular and punctual time and attendance, and adherence to The Foundling’s and your department’s specific time and attendance policies and protocol.

  • Appropriate engagement with support departments to ensure appropriate and timely hiring, performance management, investigation, discipline, suspension, and restructuring decisions concerning direct reports.

  • Any other duties and / or tasks that may be assigned
