Job summary

Integritus Healthcare is seeking a dedicated CNA to join our East Longmeadow team. You will provide quality nursing care, support residents according to care plans, and document observations in the electronic health record.

Responsibilities

• Provides quality nursing care to residents in an environment that promotes their rights, dignity, freedom of choice and individuality.
• Demonstrates respect for diversity among residents, families, caregivers, etc.
• Follows individualized care plans and supports residents accordingly.
• Assists with activities of daily living and daily care tasks as per care plan.
• Documents ADLs in electronic health records.
• Observes and reports changes in residents' condition promptly.
• Maintains resident rights and confidentiality.
• Assists residents to appointments and escorts to activities when needed.
