Job Title: CertifiedNursing Assistant

Reports To: Directorof Nursing

FLSA Class: Non-exempt

Summary of Position

Clinical participant in the Person Centered Care operationalteam. Ensures activities of daily livingare delivered according to the wishes of each resident. Actively communicates in the care planprocess via effective communication, implementation and evaluation of the carereceived by each resident.

Essential Functions

• 1. Understand the importance of customerservice and be willing to learn resident/relative satisfaction techniques as aparticipant in the Person Centered Care program. Accepts the role of a CNA in the ability ofeach resident to reach his/her highest level of physical and psychosocialwell-being. Every resident to be treatedin a dignified manner and as an individual.
• 2. Perform basic nursing skills safely andefficiently as outlined in facility educational program and nursing policiesand procedures. Thoroughly understandspolicies concerning: prohibition ofresident abuse; fall prevention, incontinence care, nutritional program;elopement protocols, skin care and pain management.
• 3. Document in the nursing assistant notesthe care and treatment provided to the resident and the resident's response orlack of response to care provided as required by the facility.
• 4. Complete food percentage, intake andoutput and bowel and bladder records.
• 5. Participate in fine dining experiencefor residents by ensuring each person is ready for meals, i.e. face and hands washed, teethbrushed, hair brushed, etc. Assist withtransport to dining room as necessary.Assist dietary server with beverages, feed dependent residents andassist with transport of resident as needed from dining room.
• 6. Provide incontinent care as perfacility protocol, with emphasis on toileting per each resident’s normalschedule and maintaining B&B continence.
• 7. Perform restorative nursing as outlinedin each resident’s plan of care.Includes ambulation, range of motion, etc.
• 8. Make sure all residents are offerednourishment during the day as requested and an evening snack ifappropriate. Document intake of snacksif such is careplanned. Ensure food andsupplies in satellite kitchen are consumed by residents only.
• 9. Perform tub baths, showers or bed bathsas per the careplan and resident choice.Understand philosophy of Bathing Without a Battle and goal of bathing isfor resident to remain free from body odor and skin breakdown. Report any skin issues noted during bathingto charge nurse.
• 10. Turn and reposition all dependent bed andchair residents as required.
• 11. Empty and record all urinary drainageunits.
• 12. Assist in dressing and undressingresidents as necessary and assist residents in hygiene related activities including oral careand denture care. Allow residents tochoose clothing to be worn on adaily basis.
• 13. Understandevery member of Person Centered Care team is responsible for assistance with activities. Any spare time of the CNA is spent with aresident, whether it be talking, reading,etc.
• 14. Assistresidents in preparing for medical tests, social activities, appointments, etc.and assist in transporting residents to and from various location.
• 15. Provide post‑mortem care as instructed.
• 16. Report incidents, including near missesand changes in residents’ condition to charge nurse or Dir. of Nursesimmediately.
• 17. Identify safety hazards and emergencysituations, report them, and initiate corrective action. Understand it is the duty of each and everyemployee to know how to safely operate equipment.
• 18. Participate in resident care conferences,assist in completion of Minimum Data Set (MDS) and deliver nursing care as perthe careplan. Remind new employees ofthe important of careplans in the Person Centered Care operational model.
• 19. Follow facility procedures for anddocument admission, discharge, and transfer of residents.
• 20. Participate in facility education programsand assume responsibility for achieving the mandatory 12 hours of in-servicesper year.
• 21. Observe facility infection controlprocedures, disaster preparedness procedures and Corporate compliance rules.
• 22. Observe Resident's Rights policies, HIPAArules and prohibition of abuse/neglect.Understand every employee of a skilled nursing facility is a mandatedreporter.

Special Requirementsof the Position

This position requires an ability to safely lift andtransfer residents weighing an average of 150 pounds with or withoutassistance, on an occasional basis; and an ability to push residents inwheelchairs up to 250 pounds on a frequent basis approximately 100 feet.
