Qualifications

• Current Ohio CNA certification is required.
• Valid driver's license with a clean driving record is required.

Responsibilities

• Safely transport residents to and from medical appointments.
• Assist residents with transfers and wheelchair transport.
• Ensure timely arrival and return from appointments.
• Maintain transportation logs and documentation.
• Keep vehicle clean, organized and inspection-ready.
• Communicate appointment updates with nursing staff and families.
• Provide CNA support on the floor when transportation duties are complete.
