Job summary

Qualifications

• High School Diploma or equivalent.
• Active Certified Nursing Assistant.
• Has 2 years of experience in a social or recreational program within the last 5 years, one of which was full-time in a therapeutic activities program.
• Proficient in the use of a personal computer.

Responsibilities

• Assist with activity programs as planned to meet patients' needs and interests.
• Complete records indicating patient attendance and participation accurately.
• Complete assessments, MDS, care plans, and gather information to design activities.
• Escort patients to and from activities and assist in developing Activity calendars.
• Provide personal care and services such as ADLs, mobility, nutrition, and hydration.
• Obtain and record vital signs and weight as assigned.
• Document all care accurately per policy and regulations.
• Maintain confidentiality and protect patient information.
• Adhere to professional standards and facility policies.
