Job summary

Qualifications

Responsibilities

• Schedule medical appointments and coordinate details with residents, families, nursing staff, and providers
• Maintain the appointment and transportation calendar and prepare needed paperwork
• Drive residents safely to and from appointments and assist them in and out of the vehicle
• Keep transportation records accurate and the facility vehicle ready for use
• Work on the floor as a CNA when needed, providing hands‑on resident care and supporting the nursing team
