Job summary

Harmony Senior Services in State College, PA is seeking a Certified Nursing Assistant to provide high-quality care and assist residents with activities of daily living. You will support independence and help residents participate in community activities, while conducting meaningful programming and maintaining care documentation.

You will collaborate with the nursing team, respond to calls and emergencies, and help with move-in transitions, all within a respectful, confidential setting.

Qualifications

  • Must possess current CNA license from the State where the community is located.
  • Effective verbal and written communication.
  • Organized and able to work well with all levels of personnel.
  • Contributing member of nursing team and follow supervisor instruction.
  • Polite and considerate of residents, visitors, and co-workers.
  • Able to walk and stand for extended periods.
  • Must lift 50 lbs and push or carry 25 lbs.
  • Safely lift and transfer residents with proper body mechanics.
  • Uniform during working hours with name badge and a smile.
  • CPR/First Aid certified or willing to become within 60 days.
  • Represent the community in a positive and professional manner.
  • Attend and complete in-services and staff meetings.
  • Willing to work a flexible schedule including weekends and holidays.

  • Follow confidentiality policies regarding release of resident information.
  • Assist residents to and from dining and activities; check attendance and status.
  • Communicate status and upcoming information to appropriate staff and families.
  • Assist new residents with move-in transition and provide emotional support.
  • Document resident status and changes in logs and records.

Job Description
  • The CNA/Direct Care Giver provides quality care and assistance with activities of daily living to residents of the community.
  • The CNA encourages residents to function at their highest level of independence and to participate in community activities.
  • Conducts activities and provides for a meaningful experience for all residents.
  • Must maintain a working knowledge of all policies and procedures pertaining to the position
  • Assists residents with activities of daily living
  • Reports incidents involving residents to include care concerns or issues, to appropriate individuals (ex: HCC, RN/LPN, or ED) and completes appropriate documentation
  • Perform housekeeping duties, such as making resident beds, cleaning resident apartments, common areas, dining room, etc. as directed by supervisor
  • Respond promptly to all resident calls for assistance or emergency alerts
  • Must be able to remain calm and competent during emergencies
  • Assists in planning and conducting activity programs for residents
  • Follows confidentiality policies regarding release of resident information
  • Assists residents to and from the dining room, and to and from activities
  • Checks on residents, ensures attendance at meals and activities
  • Checks on residents who are ill and reports status or concerns to appropriate individuals documents appropriately when necessary
  • Communicate verbally and in writing to report resident status, upcoming community information, resident appointments, admissions, and transfers
  • Assists new residents with move-in transition: provide emotional support
  • Reports all resident changes and interventions and interventions in the Daily Log and clinical record.
  • Notifies family as directed.
  • Completes all other duties as assigned
Requirements
