Job summary

Citadel Healthcare is seeking a Certified Nursing Assistant (CNA)/Recreational Services Aide to join our team in Rockford. You will assist in planning, implementing and evaluating recreational, physical, emotional and psychosocial programs aligned with each resident's care plan.

We value empathy, teamwork and a commitment to resident well‑being. A high school diploma is required, with CNA licensure and CPR certification preferred.

Qualifications

• Must be able to read, write and communicate in English.
• Strong verbal and written communication skills to lead activities.

Responsibilities

• Implements activity programming to meet resident needs and interests.
• Assists residents to and from activity programs and outings.
• Maintains rapport with families and encourages family involvement.
• Provides input to assessments of resident participation.
• Communicates concerns and responses to interventions to supervisor.
• Supports resident preferences and routines.
• All other duties as assigned.
