Job summary

Optum is seeking a Home Health Aide to provide personal care and support with daily activities to help individuals remain in their own homes. Responsibilities include assisting with bathing, grooming, dressing, ambulation, meal prep, and accompanying clients to appointments, while monitoring health and ensuring safety.

The role requires a driver's license with reliable transportation, 1 year of direct client care experience or equivalent nursing student training, and a current Texas Nurse Aide

Qualifications

• Must have a valid driver’s license with reliable transportation.
• Minimum 1 year full-time direct client care in a hospital/nursing facility OR recent (within 5 years) direct client care in an agency setting, or approved nursing student training.
• Be on the Texas Nurse Aide Registry with no abuse/neglect findings and not listed on the Employee Misconduct Registry.

Job description

The Home and Community Bases Services Aide provides support, assistance with personal hygiene and household functions so individuals can remain in their own homes.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use the EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
• Function in any home situation regardless of client age, race, creed, color, sex, disability, or financial condition.

Required Qualifications

• Current driver’s license, vehicle insurance, and reliable transportation or access to public transportation.
• Home Health Aide experience: a minimum of one (1) year full‑time experience in direct client care in a hospital or nursing facility, OR one (1) year full‑time experience within the last five (5) years in direct client care in an agency setting, OR documented completion of a training and competency evaluation program with home health experience within the last twenty‑four (24) months, OR nursing student with documentation confirming competency in basic nursing skills aligned with the school’s curriculum; OR be on Texas Department of Human Services Nurse Aide Registry with no findings of client abuse or neglect or misappropriation of client property, and must not be listed on the Employee Misconduct Registry.

Preferred Qualifications

• Current CPR certification or ability to complete within 90 days of hire.
• Six or more months of home care experience.
• Ability to work flexible hours and independently.

Pay & Benefits

The hourly pay for this role ranges from $16.75 to $25.10 per hour based on full‑time employment. All hourly wages comply with applicable minimum wage laws.
