Job Overview

The Care Partner at SEMHaven works cooperatively and respectfully with residents, coworkers, families, visitors and volunteers to create a positive and pleasant environment in the home. He or she provides for the physical, social and environmental needs of residents according to facility policies, procedures, quality improvement guidelines and local, state and federal regulations. The Care Partner actively promotes the resident’s opportunities to engage in normal life pursuits and to improve or maintain functioning levels. The Care Partner is a participant in resident‑centered care, team‑based management, and is involved in daily life activities in his or her assigned household(s). The focus of SEMHaven is to be a resident‑centered organization based on resident choice and to provide support for each member of the community.

Responsibilities

• Assist with food preparation and serving.
• Pick up clutter, remove trash, and clean spills.
• Transport residents as needed.
• Attend household meetings and learning circles.
• Fulfill simple requests for residents.
• Maintain life on the household in a manner that upholds respect, dignity, and care.

Essential Physical Demands

• Mobility – Able to move freely about the facility.
• Lifting – Regularly lift up to 50lbs.
• Climbing/Balancing – Regularly climb stairs and balance.
• Stoop/Bending – Regularly stoop or bend.

Required Qualifications

• Free from illegal drug use and from the effects of drug or alcohol use in the workplace.
• Patience and strong listening skills.

• Interpersonal skills with residents, coworkers, family members, doctors, volunteers, and visitors.
• Respect for all people and for facility possessions, keeping living and work areas clean and orderly.
