Qualifications

• High School diploma or equivalent.
• 1 year of direct patient care in any clinical setting, or in-home health/hospice is preferred.

• Ability to learn and document in EPIC charting system
• Good driving record, as defined by Presbyterian, access to a vehicle, driver’s license, auto insurance

Responsibilities

• Works clinically under the direction of a registered nurse or licensed clinician while independently providing care in the patient home setting.
• Plans and navigates route to patient service address based on road/traffic conditions utilizing driving applications when necessary.
• Manages and maintains patient caseload and patient scheduling to ensure frequency/duration of ordered care is delivered timely while taking into account patient preference.
• Monitors, collects, and records information in the patient record. Collects and records vital signs, skin, bowel and bladder function, tolerance to activities, and other patient data, as ordered in the CNA careplan.
• Reports any significant problems or changes in patient status to the primary RN/clinician.
• Performs personal care and assists with activities of daily living such as personal hygiene and emotional support.
• Assists with restorative aide duties for feeding patients and report on tolerance.
• Performs basic wound care and catheter maintenance, as ordered and directed by the RN case manager
• Applies oxygen delivery devices as ordered without altering settings or determining rates.
