Job summary

Qualifications

• High School Diploma or equivalent.
• Current Certified Nursing Assistant (CNA) certification in the state of practice.
• Current BLS certification preferred or required per facility policy.
• Recent LTC/SNF, rehabilitation, hospital, or acute care experience preferred.

Responsibilities

• Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, feeding, and mobility.
• Obtain and document vital signs, weights, intake/output, and other observations.
• Assist with transfers, positioning, ambulation, and range-of-motion exercises.
• Report changes in patient condition to the licensed nurse promptly.
• Maintain accurate and timely documentation of patient care.
• Ensure a clean, safe, and comfortable environment following infection prevention standards.
• Collaborate with nurses and healthcare team to support quality care.
• Maintain patient confidentiality and HIPAA compliance.
• Perform other duties as assigned.
