General Job Summary

Coordinate and deliver patient care under the direction of registered nurse and follow all established standards of practice within State and Federal guidelines. Act as preceptor to new staff and/or students as requested. Adhere to all Stamford Health’s policies and procedures, including the Code of Conduct and Corporate Compliance Program. Provide safe and therapeutic care using the values inherent in our hospital culture.

  • Provides direct patient care to a select group of patients and advocates on their behalf, including documenting all activities in EMR according to defined guidelines.
  • Documents pertinent information, observations, and all aspects of patient care clearly, concisely, and in a timely manner.
  • Provides care to patients including but not limited to assistance with ambulation and providing comfort items.
  • Performs bathing, dressing/undressing, bed pans/urinals (toileting), mouth care/grooming and shaving as directed by registered nurse.
  • Obtains vital signs, weights, empties/maintains drainage devices, measures intake and output, as directed by registered nurse.
  • Proactively rounds on patients at regular intervals to address needs and answer call bells.
  • Assists with meal distribution.
  • Assists with preparing patient for procedure, collecting food trays, providing water pitchers, and assists with feeding as guided by licensed nurse.
  • Changes bed linens and ensures room is tidy.
  • Assists with transferring patient (bed to chair/chair to bed), ambulation, turning and positioning.
  • Assists in transporting patients.
  • Monitors skin condition and reports to licensed nurse.
  • Assists with post‑mortem care.
  • Restocks supplies as needed to work area, treatment, and patient rooms.
  • Directs visitors as needed.
  • Manages multiple competing tasks, develops effective plan for accomplishing daily tasks, and manages priorities and allocation of available resources along with care team.
  • Practices and promotes patient safety and quality of care with continuous communication with registered nurse.
  • Alternate responsibilities may include cross‑training and floating to other identified patient care departments.
  • Adheres to department and hospital‑specific dress code policy.
  • Provides constant observation & remote video monitoring as required/assigned.
  • Assists with adjunct therapies such as enemas, heat packs, ice packs, sitz baths as directed by registered nurse.
  • Assists with wound/dressing changes and applies skin protection devices as directed by registered nurse.
  • Assists with admissions and discharges.
  • Performs phlebotomy as directed and as per policy.
  • Performs capillary blood glucose monitoring according to policy (finger sticks) and reports to registered nurse as appropriate.
  • Performs EKGs.
  • Assists with collection of information regarding patient’s physical and psychosocial condition and reports to licensed nurse.
  • Performs Unit Coordinator responsibilities as assigned.
  • Supports and promotes hospital‑wide and unit‑specific initiatives and goals (e.g., Planetree, Magnet, purposeful rounding etc.).
  • Assists with education for patients and families, including use of the Get‑Well Network.
  • Provides hand‑off report to oncoming CA.
  • Specific to Office
  • Prepares treatment rooms prior to patient arrival.
  • Assists in the collection and recording of information including current medications, allergies, and reports results to licensed nurse.
  • Checks in patients in the EMR upon arrival, witnesses signature, applies patient identifying bracelet after using two forms of identification.
  • Ensures adequate information is available for the RN prior to initial consultation by obtaining medical records timely, obtaining reports and other information as requested.
  • Answers phone calls and manages daily activity of the office under the direction of the licensed nurse.
  • Schedules repeat and new appointments for patients.

  • Respects and protects the patient’s rights to confidentiality and privacy and discloses information only for professional purposes with collaboration of registered nurse.
  • Performs service recovery and communicates as a care team to achieve the desired patient outcome.
  • Collaborates with patient, family, and health‑care team in implementing plan of care as a liaison of the hospital, clinic, and health‑care team.
  • Maintains high standards of service to patients, physicians, and other hospital departments.
  • Displays a positive attitude and works to achieve an environment of respect, accountability, teamwork, integrity, and compassion. Does not support or take part in disruptive or negative behavior.

  • Communicates any observed changes in clinical presentation to registered nurse.
  • Complies with hospital attendance policy and department staffing policy and rotations.

  • Communicates effectively with all members of the health care team and related departments to provide accurate information about the care of the patients.
  • Keeps all unit caregivers informed of issues that affect patient care or nursing practices by actively participating in shared governance.

  • Provides clear direction and collaborates responsibly with care team, when appropriate.
  • Performs duties as assigned or requested to maintain a high level of service.
  • Technical Knowledge
  • Utilizes, navigates and documents in an accurate and timely manner on the electronic medical record.
  • Maintains equipment and utilizes supplies in a safe, cost‑effective manner.
  • Completes required continuous training and education, including department specific requirements.
