Job summary

Monarch Healthcare Management is seeking a part-time CNA to provide direct routine care to tenants in alignment with plans of care. You will work under supervision of a licensed nurse and the clinical manager to support daily living activities while addressing residents' mental health and social needs.

The role emphasizes promoting independence, teamwork within the interdisciplinary care team, and maintaining the tenants' dignity and well-being in a supportive environment.

Qualifications

• Current certification and listing on the Minnesota Nursing Assistant Registry.
• Ability to clearly speak and proficiently read and write English.

Responsibilities

• Provide tenants with high-quality direct routine care in activities of daily living.
• Work under supervision of licensed nurse or AL Clinical Manager and in accordance with the Plan of Care and QAPI program.
