Responsibilities

• Provide direct patient care to residents, including assisting with ADLs (bathing, dressing, grooming).
• Monitor and report changes in resident condition to nursing staff and other healthcare professionals.
• Assist with medication administration, as needed.
• Maintain accurate and up-to-date records of resident care and progress.
• Collaborate with the interdisciplinary care team to develop and implement individualized care plans.
• Participate in ongoing education and training to improve skills and knowledge.
