Job summary

Amedisys is seeking a PRN Hospice Aide to provide compassionate in-home care to patients under the direction of a registered nurse. Responsibilities include personal care, safety adherence, meal assistance, light housekeeping, and documenting patient status during visits.

The role requires a High School diploma, active CNA certification, CPR, and a valid driver’s license. CNA experience in the last two years and reliable transportation are needed to support patients where they live.

Qualifications

Responsibilities

• Provides personal care services to patients.
• Applies safety principles and proper body mechanics to support care techniques, ambulation, transfers, ROM and positioning, and household tasks.
• Plans and prepares nutritious meals and assists with feeding.
• Performs homemaking and other environmental services as assigned.
• Observes, reports and documents changes in patient status; reports to the primary nurse when task cannot be performed.
• Follows written care instructions provided by the nurse prior to service delivery.
• Assists patients with prescribed exercises as instructed by the nurse or therapist.
• Performs and records simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned.

• Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of clients, transferring clients, assisting with the normal range of motion and positioning, and doing household chores.
• Plans and prepares nutritious meals, assists with feeding, as assigned.
• Performs homemaking and other environmental services as assigned.
• Observes, reports, and documents any changes in patient’s status. If unable to perform a certain task, report to the primary nurse.
• Follow specific written instructions for personal care prepared by a registered nurse in accordance with the plan of care. Written instructions are to be received prior to the delivery of services.
• Assists patients with prescribed exercises as instructed by nurse or therapist.
• Performs and records simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned.

Qualifications

• High school diploma or equivalent.

• Current state certification (or registration) as a nurse assistant as per state certification/registration requirements.
• CNA experience within the last two years.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
