Job summary

UnitedHealth Group is seeking a Per Diem CNA Hospice Aide to deliver compassionate, hands-on care to patients and support for families during end-of-life care. You will assist with daily living activities and promote comfort, dignity, and emotional well-being according to the plan of care.

Working with the hospice team, you will follow written instructions from a registered nurse, document changes in patient status, and provide respectful, individualized care.

Qualifications

• Current state certification (or registration) as a nurse assistant (CNA) per state requirements.
• Current CPR certification.
• Valid driver’s license, reliable transportation and liability insurance.

Responsibilities

• Provides personal care services to patients.
• Applies safety principles and body mechanics for transfers, ambulation, positioning, and light housekeeping.
• Plans and prepares nutritious meals, assists with feeding.
• Performs homemaking and other environmental services as assigned.
• Observes and reports changes in patient status; reports to the nurse when unable to perform tasks.
• Follows written care instructions prepared by a registered nurse.
• Assists patients with prescribed exercises as instructed by nurse or therapist.
• Performs and records simple urine tests for glucose, ketones, and protein.

As the Per Diem CNA Hospice Aide, you will play a vital role in providing compassionate, hands‑on care to patients and support for their families during end‑of‑life care. This position focuses on assisting patients with daily living activities while promoting comfort, dignity, and emotional well‑being. Working closely with the hospice care team, as the CNA Hospice Aide, you will help ensure patients receive respectful, individualized care in alignment with their care plan.

Primary Responsibilities:

• Provides personal care services to patients
• Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of clients, transferring clients, assisting with the normal range of motion and positioning, and doing household chores
• Plans and prepares nutritious meals, assists with feeding, as assigned
• Performs homemaking and other environmental services as assigned
• Observes, reports, and documents any changes in patient's status. If unable to perform a certain task, report to the primary nurse
• Follow specific written instructions for personal care prepared by a registered nurse in accordance with the plan of care. Written instructions are to be received prior to the delivery of services
• Assistspatientswith prescribed exercises as instructedbynurse or therapist
• Performs and records simple urine tests for sugar, acetone, and albumin
• Other duties as assigned

Required Qualifications:

• Current state certification (or registration) as a nurse assistant (CNA) as per state certification/registration requirements
• Current CPR certification
• Valid driver's license, reliable transportation and liability insurance

Preferred Qualifications:

• CNA experience within the last two years

The hourly pay for this role will range from $16.00 to $23.85 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
