Job summary

Amedisys Home Health is seeking a compassionate hospice aide for part-time day shifts to support patients at home. You will provide personal care, assist with daily activities, and help create a comfortable, safe living environment.

This role emphasizes dignity, safety, and teamwork with registered nurses. You will follow plans of care, document changes, and encourage patients and families as they navigate home-based hospice care.

Qualifications

• CNA experience within the last two years is required.
• High school diploma or equivalent is required.
• Current state CNA certification and CPR certification are required.

Responsibilities

• Provides personal care services to patients.
• Applies safety principles and body mechanics during care and assistance with ambulation, transfers, ROM, and positioning.
• Plans and prepares nutritious meals and assists with feeding.
• Performs homemaking and environmental tasks as assigned.
• Observes and reports changes in patient status to the nurse.
• Follows written instructions for care from the RN.
• Assists with prescribed exercises as directed by the nurse or therapist.
• Performs simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned.

Responsibilities

• Provides personal care services to patients.
• Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of clients, transferring clients, assisting with the normal range of motion and positioning, and doing household chores.
• Plans and prepares nutritious meals, assists with feeding, as assigned.
• Performs homemaking and other environmental services as assigned.
• Observes, reports, and documents any changes in patient\'s status. If unable to perform a certain task, report to the primary nurse.
• Follow specific written instructions for personal care prepared by a registered nurse in accordance with the plan of care. Written instructions are to be received prior to the delivery of services.
• Assists patients with prescribed exercises as instructed by nurse or therapist.
• Performs and records simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned.

Qualifications

• High school diploma or equivalent.
• Current state certification (or registration) as a nurse assistant as per state certification/registration requirements.
• CNA experience within the last two years.
• Current CPR certification.
• Valid driver\'s license, reliable transportation and liability insurance.
