Job summary

Amedisys, a leading home health and hospice provider, is seeking a hospice aide to deliver compassionate personal care to patients at home across Downtown San Antonio, Medical Center, and Westside. This full-time role requires a current CNA license, CPR certification, and reliable transportation to support patient needs.

You will follow care plans, assist with activities of daily living, monitor changes in patient status, document observations, and collaborate with nurses and therapists to help

Qualifications

• High school diploma or equivalent.
• Current state CNA certification.
• CNA experience within the last two years.
• Current CPR certification.

Responsibilities

• Provides personal care services to patients.
• Applies safety principles and proper body mechanics during care and activities of daily living.
• Plans and prepares nutritious meals and assists with feeding.
• Performs homemaking and environmental tasks as assigned.
• Observes and reports changes in patient status to the nurse.
• Follows written care instructions prior to service delivery.
• Assists with prescribed exercises as directed by nurse or therapist.
• Records simple urine tests as instructed and performs related tasks.
• Other duties as assigned.

Responsibilities

• Provides personal care services to patients.

• Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of clients, transferring clients, assisting with the normal range of motion and positioning, and doing household chores.
• Plans and prepares nutritious meals, assists with feeding, as assigned.
• Performs homemaking and other environmental services as assigned.
• Observes, reports, and documents any changes in patient's status. If unable to perform a certain task, report to the primary nurse.
• Follow specific written instructions for personal care prepared by a registered nurse in accordance with the plan of care. Written instructions are to be received prior to the delivery of services.
• Assists patients with prescribed exercises as instructed by nurse or therapist.
• Performs and records simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned.

Qualifications

• High school diploma or equivalent.

• Current state certification (or registration) as a nurse assistant as per state certification/registration requirements.
• CNA experience within the last two years.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
