Amedisys is seeking a hospice aide to join our team in Media, PA, offering full-time day shifts. The role emphasizes in-home care and compassionate support for patients in a home setting.

Qualifications

• Current state CNA license required.
• CNA experience within last two years strongly preferred.
• Current CPR certification required.

Responsibilities

• Provides personal care services to patients.
• Applies safety principles and proper body mechanics during activities of daily living.
• Plans and prepares nutritious meals and assists with feeding.
• Performs homemaking and environmental services as assigned.
• Observes and reports changes in patient status to the nurse.
• Follow written instructions from a registered nurse.
• Assists with prescribed exercises as instructed by a nurse or therapist.
• Performs and records simple urine tests for sugars, ketones, and albumin.
• Other duties as assigned.

Responsibilities

• Provides personal care services to patients.
• Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of clients, transferring clients, assisting with the normal range of motion and positioning, and doing household chores.
• Plans and prepares nutritious meals, assists with feeding, as assigned.
• Performs homemaking and other environmental services as assigned.
• Observes, reports, and documents any changes in patient's status. If unable to perform a certain task, report to the primary nurse.
• Follow specific written instructions for personal care prepared by a registered nurse in accordance with the plan of care. Written instructions are to be received prior to the delivery of services.
• Assists patients with prescribed exercises as instructed by nurse or therapist.
• Performs and records simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned.

Qualifications

• Homehealth/hospice experience preferred within last 12 months*
• High school diploma or equivalent.
• Current state certification (or registration) as a nurse assistant as per state certification/registration requirements.
• CNA experience within the last two years.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
