Job summary

JourneyCare Hospice is seeking Home Health CNAs to join our growing team in Long Grove, IL. Competitive salaries, immediate openings, and comprehensive benefits are offered. The role features a Monday–Friday daytime schedule with no weekends or holidays.

Qualifications

• Must be certified by the state as a nurse aide.
• Must be listed on the state aide registry with a clear record.
• Ability to deliver patient care.
• Must be 18 years of age or older as required by state law.
• Valid driver's license with insured vehicle.

Responsibilities

• Giving personal care including baths, back rubs, oral hygiene, shampoos, and changing bed linens.
• Assisting in dressing and undressing patients.
• Planning and preparing nutritious meals including shopping.
• Assisting in feeding the patient.
• Providing proper care and observation of patients' skin to prevent breakdown of tissue.
• Reporting on patients' condition and significant changes to the Case Manager.
