Qualifications

• Active Wisconsin CNA certification.
• High school diploma or equivalent required.
• Excellent communication and reliable work ethic required.
• Long-term care or skilled nursing experience preferred but not required.

Responsibilities

• Provide ADL assistance with dignity and respect.
• Assist residents with transfers, mobility, and positioning.
• Monitor resident conditions and report changes to the charge nurse.
• Document care accurately in electronic health records and track vitals.
• Collaborate with nursing and clinical teams to execute care plans.
