EMPATHYHANDS HOMECARE LLC in California seeks a compassionate Home Care Aide. You will work directly with patients in their homes under a one‑on‑one care plan, delivering personalized care and building trusting relationships.

You should be organized, detail‑oriented, and able to communicate clearly with patients and families. Requirements include a Home Health Aide program completion and a High School Diploma or GED, CPR certification, and a valid driver’s license.

Qualifications

• Graduated from an accredited Home Health Aide program
• High School Diploma or GED
• Two-year prior professional experience
• Driver’s license required or ID
• CPR certification required
• Must have Current TB test
• Live Scan and register with the California Home Care Registry (HCA)

Responsibilities

• Assist patients with daily activities such as baths, transfer wheelchairs, or ambulating
• Care for patients by changing bed linens, doing laundry, cleaning the home Light housekeeping, and assisting with personal care
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
• medications reminders
• Engage patients in range of motion exercises or other activities

Skills

Job Summary

We are looking for a Home Care Aide to join our team! You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, providing personalized and attentive care and building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Care or Hoyer lift experience is a plus.
