Job summary

Bristol Hospice is seeking an experienced Certified Home Health Aide (CHHA) to join our dedicated Interdisciplinary Group Team. You will work under supervision of a Registered Nurse and perform various personal care tasks to meet the needs of patients in their homes, promoting comfort and dignity.

Ideal candidates are CNAs with HHA certification, compassionate and patient, capable of coordinating with families.

Qualifications

  • Must be a Certified Nursing Assistant (CNA) in good standing.
  • Must have a Home Health Aide (HHA) certification where required by state law.
  • Experience delivering in-home hospice or home health care is preferred.

Responsibilities

  • Provide personal care including bathing, grooming, transfers, and meal assistance.
  • Monitor patient condition and report changes to Case Manager.
  • Maintain patient safety, comfort, and dignity during care visits.
  • Document care provided and observe for any changes in health status.
  • Support families with education and emotional support during home visits.
