Job summary

Partners In Care Hospice is hiring Certified Home Health Aides (HHA) who also hold CNA credentials to support our patients in home and facility settings across Placer, Nevada, El Dorado and Sacramento Counties.

Candidates should be able to lift up to 50 lbs, pass background checks, and have a valid CA driver's license with an insured vehicle. We offer part-time/per-diem roles with paid training and mileage reimbursement.

Qualifications

• Must be a current California Certified Home Health Aide (HHA) and CNA.
• Lift up to 50 lbs and assist with mobility and transfers.
• Pass background clearance.
• Pass a work physical and TB test.
• Valid driver’s license and insured vehicle.
• Authorized to work in the United States.

Responsibilities

• Provide personal care for hospice patients in home or facility settings.
• Accurate reporting and documentation of patient observations to the nurse.

Hospice Aide

We have openings for Certified Home Health Aides who hold a HHA certificate, to perform Hospice Aide duties for our Patients. We are a hospice provider serving Placer, Nevada, El Dorado and Sacramento Counties.

Candidates will have morning availability from 7am on and be available to provide care in the home, as well as in facility settings.

We are looking for reliable, experienced, compassionate and dedicated individuals to fulfill this role. Hospice agencies are required to use certified Home Health Aides to assist with personal care for Hospice patients. An HHA has training in addition to possessing CNA (Certified Nursing Assistant) certification, usually from a vocational school or community college, and then must pass a state certification exam. Additionally, an HHA can help with dressing, toileting, and reporting changes observed with Patient condition to the nurse.The right candidates will be professional and self-motivated. They will be responsible for accurate/precise reporting, the documentation of their observations and the care performed during patient visits. The Hospice Aide provides personal care to patients, under the direction of a registered nurse.

Qualifications:

• Must be a current California Certified Home Health Aide - HHA and CNA


• Must be able to lift up to 50 lbs and be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull during the course of providing direct care, including bed bound care and transfers


• Ability to pass a background clearance


• Able to pass a work physical and TB test


• Must possess a valid driver's license


• Have a reliable, insured vehicle


• Must legally be eligible to work in the USA
