Job summary

Covenant Health HomeCare and Hospice in East Tennessee seeks a Certified Nursing Assistant to provide patient care under supervision, following policies and procedures. The Home Health Aide reports to the assigned Nursing or Hospice Clinical Supervisor.

This role travels to patient homes for daily care, vitals, mobility assistance, and routine treatments, with nursing staff support and required documentation.

Qualifications

  • High school diploma or equivalent is required.
  • CNA licensure from the State of Tennessee is required on hire.
  • CPR certification is required.
  • Out-of-state hires must have Tennessee CNA license in process at hire.

Responsibilities

  • Take and record vitals and monitor input‑output as directed.
  • Assist with daily activities, mobility, meals, and personal care.
  • Support RN/LPN in treatments and documentation.
  • Maintain infection control practices and proper equipment use.

Homecare And Hospice Overview

Covenant Health HomeCare and Hospice is the area’s largest locally owned home health and hospice provider, with one of the largest rehabilitation departments in East Tennessee. Position Summary

Provides patient care under the supervision of licensed staff and in compliance with Covenant HomeCare policies and procedures. The Home Health Aide reports to the assigned Nursing or Hospice Clinical Supervisor.

Responsibilities
  • May take and record temperature, pulse, respiration, blood pressure and input‑output measurements as outlined on the HHA POC
  • Provides morning or evening care, which may include bed bath, oral hygiene, shampooing and combing hair, back care, nail care, skin care, dressing patient, changing the bed linen and cleaning the surrounding area, and other supportive care as needed.
  • Performs routine treatment procedures as outlined on the HHA Poc within the defined scope of practice for certified nursing assistants.
  • Assists RN and/or LPN in treatment or procedures.
  • Provides general care such as positioning, lifting or turning patients; assisting in use of bedpan, urinal or commode; assist with ambulation and excercises, and safe patient transfers including use of patient lift; and application and/or removal of anti‑embolism stockings.
  • Assists in cleaning equipment and preparing for disinfecting or sterilization, and reporting equipment that is not working properly.
  • Properly uses equipment such as ice bags, ace bandages, and Abd binders as outlined on the HHa POC.
  • Performs basic infection control techniques according to Covenant HomeCare policy; examples are proper Bag Technique, hand washing, glove usage, appropriate handling and disposing of wastes or un‑sterile compresses, and keeping dirty and clean items separate.
  • Attends to nutritional needs of patient, which may include preparing meals and feeding patients.
  • Performs range of motion exercises where appropriate under supervision of Physical Therapy.
  • Checks schedule and calls patients to provide patients with arrival times.
  • Reports any changes observed in condition or behavior of patients and unusual incidents to licensed staff.
  • Assists in orienting new employees.
  • Demonstrates knowledge and competency in providing for age‑specified needs of the population served.
  • Demonstrates ability to perform skills on annual skills checklist.
  • Completes and submits patient visit documentation.
  • Assists with medications that are ordinarily self‑administered (including topical) as outlined on the HHA POC.
  • Local travel required.
  • Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
  • Performs other duties as assigned.
Qualifications
Minimum Education

High school diploma or equivalent.

Minimum Experience

S ix (6) months hospital or related experience.

Licensure Requirements

CNA Licensure from the State of Tennessee is required upon hire. Out of State hires must have the Tennessee CNA License “in process” as reflected on the state web site upon hire. CPR required. Employee must have a valid Tennessee driver’s license Class D and state mandated minimum insurance coverage. Driving record must meet Covenant Health minimum standards at the date of hire and throughout employment tenure.
