Job summary

Senior Community Care of Colorado PACE in Montrose, CO is seeking a Certified Nursing Assistant (CNA) to support participants in daily living and restorative care. This role requires travel to participant homes and entails working within an interdisciplinary team to promote safe, independent living in the community.

The position offers a Monday–Friday schedule with some weekends/holidays, and pay range of $19.50–$23.50 per hour based on experience.

Qualifications

• High School diploma or equivalent.
• Current Colorado license as Certified Nursing Assistant (C.N.A).
• Minimum of 1 year experience with frail or elderly population.

Responsibilities

• Assist participants with activities of daily living and restorative care to help them live independently.
• Monitor participants' health status and report changes to the Personal Care Supervisor.
• Travel to participant homes as required to provide care and support.
• Assist with Day Center activities, including meal service and cleanup.

Pay Range: $19.50 - $23.50 per hour (based on previous work experience)

Schedules: Monday - Friday 1:00pm - 9:30pm, occasional weekends and holidays

Travel: Requires travel to participant homes. *You must be at least 21 years of age!

What You Bring (Requirements)

• High School diploma or equivalent required.
• Current License as Certified Nursing Assistant (C.N.A) in the state of Colorado require.
• A minimum of one year’s experience in working with the frail or elderly population required.
• Must have a valid driver’s license, proof of insurance and have means of transportation.

What You’ll Be Responsible For

The Certified Nursing Assistant is responsible for providing and assisting participants with activities of daily living, restorative and supportive care necessary to the preservation of assisting participant’s in living independently in the community. Assists in monitoring and reporting the health status and providing a safe environment for the PACE participants of Senior CommUnity Care. Functions as a member of the Interdisciplinary Team. This position may require travel to participant homes.

• Assists participants with Independent Activities of Daily Living (IADL’s) including but not limited to grocery shopping, cleaning, changing bed linens, laundry, meal preparation and other routine household care as assigned. Reports changes in participant’s condition and needs to the Personal Care Supervisor (or designee).
• Assists with participant activities in the Day Center, including meal service and clean-up.
