Qualifications

Responsibilities

• Provide direct care to residents, including bathing, dressing, grooming, and toileting.
• Assist with meals, snacks, and hydration as needed.
• Monitor and report changes in resident condition to nursing staff.
• Support resident mobility, balance, and transfer activities.
• Maintain a safe and clean environment for residents.
• Collaborate with nursing staff to develop and implement individualized care plans.
• Participate in resident-centered care conferences to ensure holistic care.
• Maintain accurate and timely documentation of resident care.
