Heartlite Hospice, part of the Optum family, is seeking a Home and Community Based Services Aide to support individuals in remaining in their homes. Primary tasks include personal care, daily living activities, and accompanying clients to appointments within a compassionate, patient-centered environment.

Role requires reliable transportation and flexibility; CPR certification is preferred, with 6+ months of home care experience advised. Benefits and a supportive team culture are offered.

• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

As the Home and Community Bases Services Aide, you will provide support, assistance with personal hygiene and household functions for an individual to be able to remain in their own home.

Primary Responsibilities:

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Pay and Benefits:

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc.

The hourly pay for this role will range from $17.50 to $19.50 per hour based on full-time employment.
