Job summary

Baypark Center for Nursing & Rehabilitation in New York is seeking a Certified Nursing Assistant (CNA) to join our care team. The ideal candidate holds a CNA certificate and brings intent to contribute to patient support under nursing supervision.

Responsibilities include providing patient care per the Plan of Care, maintaining accurate notes, transporting patients as needed, and keeping the environment clean and safe. Flexible scheduling and orientation support are offered.

Qualifications

• Current CNA certificate is required.
• Resume is required.
• Physical examination.
• MMR & Varicella titers.
• PPD or chest X-ray if positive.
• 1 personal reference letter, 1 professional reference letter.
• Original Social Security Card.
• Original non-expired State ID.
• Original Proof of Citizenship/Immigration documents.

Responsibilities

• Provide patient care in accordance with nursing supervision and Plan of Care.
• Manage your own patient caseload efficiently.
• Complete timely and accurate clinical notes addressing progress.
• Assist in transport of patients.
• Maintain a clean and healthy environment.
